Tobacco Definition Preferred Plus Nicotine Non-User: NO use of tobacco or nicotine products in the past 60 months.

Preferred Nicotine Non-User: NO use of tobacco or nicotine products in the past 36 months.

Standard Plus Nicotine Non-User: NO use of tobacco or nicotine products in the past 12 months.

Preferred Nicotine User: Use of tobacco or nicotine products in the past 12 months.

Preferred Plus Nicotine Non-User:

Blood Pressure: History of BP treatment may or may not be acceptable. Current and historical readings may not exceed 135/85

Cholesterol: Level may not exceed 210; No history of treatment

Cholesterol/HDL Ratio: Ratio of 5.0 or less

Family History: No death in either parent or siblings before the age of 60 from heart problems, cerebrovascular accident (CVA), coronary artery disease (CAD), diabetes, or cancer (except basal cell carcinoma)

Driving Record: No record of DWI/DUI or reckless driving in the last 5 years. No more than 2 moving violations in the last 3 years.

Alcohol & Drug Use: No history of treatment by a medical professional for use of alcohol or drugs.

Mental Illness/Depression: No current or recent treatment, including counseling, for mental illness or depression.

Cancer History: No cancer history of any kind (except single episode basal cell carcinoma).

Personal Medical History: No personal history of any serious health conditions including, but not limited to: heart or vascular disease, diabetes, cancer or melanoma, disorder of lungs, liver, kidney or seizure disorder, rheumatoid arthritis, stroke, ulcerative colitis, or Crohn's disease.

Avocation: NO participation in hazardous sports or recreational activities. (e.g. Hang gliding, skydiving, mountain climbing, scuba diving, powered vehicle racing, rodeos)

Aviation: NO participation in private aviation activities. Commercial airline pilots may qualify.

Foreign Travel and Residency: No foreign residency outside of continental North America, Western Europe, Australia or New Zealand. (Short term travel outside of these countries may or may not be acceptable - a list follows which changes periodically)

Citizenship: Must be a permanent resident of the US or its territories. Applicants who are not US citizens must have Green Card or Permanent Visa and be able to supply the number.
